Abstract:

Background/Objective: Patellar osteomyelitis is an exceedingly rare pediatric condition. Its nonspecific presentation often mimics prepatellar bursitis or traumatic injuries, delaying diagnosis and treatment. This report highlights the importance of advanced imaging and multidisciplinary management in optimizing outcomes.

Methods: A 10-year-old girl presented with anterior knee pain and swelling. Laboratory tests showed elevated inflammatory markers (CRP 12 mg/L, ESR 60 mm/hr). Plain radiographs were unremarkable, but MRI confirmed patellar osteomyelitis with a surrounding abscess. Microbiological analysis identified Staphylococcus aureus as the causative organism. Treatment included a six-week antibiotic course, starting with intravenous cefazolin followed by oral cephalexin.

Results: The patient achieved complete resolution without surgical intervention. MRI findings and microbiological studies allowed precise diagnosis and targeted therapy. Six-month follow-up confirmed restored function and no recurrence.

Conclusions: This case demonstrates that early use of advanced imaging and tailored antibiotic regimens can prevent complications in rare pediatric infections. Multidisciplinary care is essential for timely diagnosis and optimal outcomes, underscoring the need for further research into rare bone infections like patellar osteomyelitis.
