Title : Femur fractures in the elderly in the SUS: Have there been changes in the pattern of hospitalizations due to the COVID-19 pandemic?

Abstract:

Proximal femoral fracture is a common and important cause of mortality and disability in the elderly In the Covid-19 pandemic social isolation was advised, especially for risk groups and reorganization of assistance. Prioritizing resources for the new problem. The motivation for this work is to find out whether this tension may have changed the profile of traditional emergency hospital care. Methodology: An analysis was carried out of the historical series of hospitalizations of patients aged 00 years and over. admitted due to proximal femur fracture in hospitals of the Unified Health System in the slate of Rio de Janeiro. with a main diagnosis of 1CD 572.0 (neck fracture). of the femur). ICD 72.1 (pertrochanteric fracture) and 1CD 72.2 (subtrochanteric fracture), from 2016 to 2022. The data refer to the Hospital Admissions System of the Unified Health SystemISIH-SUS1. There was an analysis of the temporal evolution and an analysis of the association with death in the pre-pandemic period (2016 to 2019), during the pandemic (2020 and 2021) and post-pandemic (2022). according to sociodemographic and hospitalization characteristics. A time series model was developed using joinpoint regression. Local and patient flows exported and imported to other municipalities in the state were calculated. Result: There was little change in hospital care for proximal femur fractures during the Covid-19 pandemic in the state of Rio de Janeiro. There was a slight increase in hospital transfers IN). mortality (%). conservative treatment (°.i1) and a decrease in hospitalization days from 13.5 days (average} to 10.5 days (average). And in terms of hospitalization flows. the movement between the municipality of residence and the municipality of hospitalization underwent few changes. with some small municipalities no longer having a local flow (13%) to export patients to larger municipalities, others only reduced the local flow and increased the export of patients 12S,20e1. Conclusions: Hospitalization for proximal femoral fractures in the elderly is a relevant public health issue for many countries. as mortality is still very high_ These patients became very vulnerable during the Covid-19 pandemic, as they belonged to the risk group and mortality among the elderly was very high. but care for proximal femoral fractures, being a hospital emergency. was maintained and little changed during the pandemic in the state of Rio de Janerio.
